Deeply inspired by Peter Reich's 'A Book of Dreams', the song tells the emotional story of psychoanalyst Wilhelm Reich's arrest through the perspective of his young son. The driving, rhythmic arrangement heavily utilizes the Fairlight CMI synthesizer paired with a striking, staccato string sextet. The cinematic music video, featuring actor Donald Sutherland alongside Bush, is regarded as a hallmark of 1980s narrative music videos.
